Key Questions for NHS Complaints (Public, Patients, Carers)

If you are a patient, carer, or family member, these questions help frame your formal complaint:

• Describe the issue clearly and factually. • What care or service caused concern
• Date, time, location (hospital, GP surgery, department, etc.)
• Staff members, service users, or departments (if known).
• Was there poor care, neglect, discrimination, lack of communication, or delays?
• Describe the emotional, physical, or practical impact on the patient or family.
• What was their response? Was it resolved?
• Apology, investigation, improvement in service, policy change, etc.
